Required Documentation to Submit
- Affirmation Letter from IRS confirming tax-exempt 501(c)(3) status (through your fiscal agent, as applicable)
- Project Budget (including revenues and expenses for this grant in Excel format; template provided by JFCS by separate email). [July 1, 2026 to June 30, 2027]
- Tax Documentation: Please attach a Form 990 Tax Return for the most recently completed fiscal year from your organization or fiscal agent.
